Employer Guidance for Completing Forms
These employer presentations provide direction on completing the forms ERSRI requires for a member at the time of retirement. Covers the Employer Certification of Retirement and Final Wages, Salary Verification for Service Credit, Verification of Retroactive Salary, and Teacher Day Count forms. Retirees working under Post-Retirement provisions may use this worksheet to report or track the days worked each month for their ERSRI-participating employer. THIS WORKSHEET SHOULD NOT BE SUBMITTED TO ERSRI. The retiree working Post-Retirement Employment should submit this worksheet directly to their employer, if directed.
QDRO Model - Maximum Benefit
The QDRO models listed below are for retirees who selected the maximum benefit form of payment.
QDRO Model Max State and Teachers
Use this model if you are a retired state employee or retired teacher who selected the maximum benefit form of payment, or if you are an active state employee or teacher and your ex-spouse’s benefit is to be calculated as if you selected the maximum benefit form of payment.
If you are retired, your beneficiary is still entitled to a death benefit, even if you have selected a retirement option that does not include a survivor benefit. If you do not name a beneficiary, a death benefit will be issued to your estate. To name a beneficiary, you can submit a completed and notarized Beneficiary Designation Form to ERSRI.

How to Review Your ERSRI Beneficiary Information Online
For members who have registered for online access to their retirement accounts, you can review your designations by simply logging into the ERSRI Member Portal, navigating to the “My Pension” section, and selecting “View My Pension Profile”.
Updating Your ERSRI Beneficiary
You may change or update your beneficiary for the death benefit payment at any time by downloading and completing the Beneficiary Designation form below, which contains detailed step-by-step instructions. Please note that this form must be notarized before it is submitted to ERSRI.
Completed forms may be sent to ERSRI by mail or delivered to the address listed on the form. Beneficiary forms submitted by fax or email will not be processed.
If you do not have access to a printer, please call the Member Service Center to request that this form be sent to you by mail.
If you selected a joint and survivor pension payment option (Option 1 or Option 2) at the time of your retirement, you may not change the beneficiary you selected to receive your ongoing pension payment after your death.
Withdrawing Your Contributions
Upon termination of employment, if you are not vested in the system or if you are vested but elect not to receive a deferred retirement benefit from ERSRI, you can elect to take a refund of your pension contributions without interest. You can roll this money over into a qualified retirement account or take the refund as a direct cash payment.
If taken in the form of a direct cash payment to you (in lieu of a roll over), a refund of your contributions is subject to state and federal income taxes, and ERSRI must withhold 20% in federal tax from the taxable portion of your refund. In addition, if you receive a lump sum distribution of your pre-tax member contribution account from the System before the minimum age for federal and state requirements, and you do not roll it over into another qualified plan or an Individual Retirement Account (IRA), you may also have to pay a tax penalty for early withdrawal.
When considering a refund, ERSRI strongly encourages you to seek advice from a qualified tax professional. ERSRI cannot provide you with such advice.

Use this form to receive information regarding a refund of contributions.
If you wish to withdraw funds from your Defined Contribution Plan after you terminate your employment, or if you have related questions, contact TIAA at (800) 897-1026 or their website by using the link below. Note that your employer must terminate you in the TIAA system before you can take a refund, which may take up to 8 weeks from your last day of work.
