Part-Time Teacher Verification: Use this form to purchase credit for time you worked as a part-time teacher.

Private Teaching Credit: Use this form to purchase credit for time you worked as a private school teacher.

Substitute Teaching Credit: Use this form to purchase credit for time you worked as a substitute teacher.

Official Layoff Verification: Use this form to verify and purchase time during which you were officially laid off from your position.

Official Leave Verification: Use this form to verify and purchase time during which you were on official leave from your position.

Prior Time Verification: Use this form to verify and purchase time during which you worked, but did not make contributions to the retirement system.
